Main tasks:
Organising and assessing care for people losing their independence in their own homes:
- Carry out a diagnosis of each situation during home visits (return from hospitalisation, significant deterioration in health, ....),
- Contacting service providers to set up individualised assistance plans,
- Coordinate the various interventions according to the needs and expectations of beneficiaries (in particular through coordination meetings),
- Carry out reassessment visits,
- Taking part in interdisciplinary meetings with beneficiaries, with the aim of improving our practices.
Administrative tasks :
- Compile and complete beneficiary files in line with the requirements of the subsidising authority,
- Ensure optimum communication of messages using ad hoc software
- Taking an active part in team meetings and showing an interest in the various projects developed by the ASD.
The profile we are looking for :
- You have a higher education qualification with a paramedical focus: social worker, graduate social nurse or graduate nurse specialising in community health or public health.
- You can easily get to our head office in Verviers
- You enjoy working in a team
- Your advantage: you have experience in home help
- You have a good command of basic IT tools (Word, Outlook, Excel)
- You have good listening, summarising and communication skills, both verbal and written.
- You know how to strike a balance between autonomy and teamwork.
